Application Process

Thank you for your interest in becoming a Portneuf Volunteer! Because we work with patients and there are privacy rules and regulations, we need each prospective volunteer to to complete the three part application process. Please complete the application process that fits you best:
Adult/ Non-Student Application
  1. Adult/Non Student Volunteer Application
  2. Medical & Health History
  3. Read the Volunteer Training Booklet & take the Online Test (results are emailed to the Director of Volunteer Services) and will be reviewed during your volunteer interview)
    • The Post Test - pages 37-44 - are in the booklet, but can be completed online
College Student Application
  1. College Student Volunteer Application
  2. Medical & Health History
  3. Read the Volunteer Training Booklet & take the Online Test (results are emailed to the Director of Volunteer Services) and will be reviewed during your volunteer interview)
